Hannah Tolar, FNP-C, developed a passion for nutrition and wellness through her work caring for patients with chronic health conditions and helping individuals navigate their weight-management journeys. She has seen firsthand how nutrition, lifestyle habits, medications, and access to supportive care can significantly affect a person’s overall health and quality of life.
As a board-certified family nurse practitioner, Hannah has experience in primary care, chronic disease management, medical weight management, and preventive wellness. Her clinical background allows her to support individuals living with conditions such as obesity, diabetes, high blood pressure, high cholesterol, cardiovascular disease, and other metabolic health concerns. She is especially passionate about weight management, women’s health, hormonal health, and helping clients develop realistic habits that can be maintained long term.
Hannah believes everyone deserves compassionate, individualized healthcare without judgment or shame. She understands that health and weight are influenced by much more than willpower alone. Genetics, medical conditions, medications, hormones, stress, lifestyle, and access to resources can all play an important role. Her approach is collaborative, encouraging, and centered around meeting each person where they are.
Hannah enjoys helping clients better understand their health, feel confident asking questions, and create realistic goals that fit into their everyday lives. Rather than focusing on perfection or restrictive plans, she believes in sustainable changes that support both physical and emotional well-being. Her goal is for every client to feel heard, respected, and supported throughout their healthcare journey.
Hannah earned her Family Nurse Practitioner degree from Chamberlain College of Nursing. She has worked in skilled nursing, long-term care, telehealth, primary care, chronic disease management, and medical weight-management settings.
Outside of work, Hannah enjoys spending time with her husband and son, staying active, participating in fitness activities, and being involved in her community. She understands how challenging it can be to balance health goals with work, family, and daily responsibilities, and she strives to help clients find an approach that feels practical, supportive, and sustainable.
